Create or open the quiz in NotebookLM that you want to turn into a Google Form. Check that every question and answer choice is finished before you start.
Install the NotebookLM quiz exporter extension in your browser, then sign in to the Google account whose Drive should hold the finished form.
Open the quiz in NotebookLM, click the Export button and choose Google Forms from the list of formats. One click starts the export.
Let the export run. The extension copies the quiz title, questions, answer choices and correct answer key into a new form.
Open Google Drive, find the new form and open it. Read through the title and questions and fix any wording that came across oddly.
