Steps
Open forms.google.com in a browser and sign in with your Google account. Click the Blank quiz template to start a new assessment.
Click Untitled form and type a clear title, such as Science Quiz 1. Add the number of items and any instructions in the description box.
Type your first question in the Untitled Question field. Pick the type from the dropdown on the right, such as multiple choice, checkboxes or short answer.
Click the plus button on the floating toolbar to add each new question. Use the duplicate icon when several items share the same answer choices.
Switch on Required at the bottom of every question so students cannot submit the form with blanks.
Open the Settings tab and turn on Make this a quiz. Choose when marks are released and whether students see missed questions or correct answers.
Go back to the Questions tab, click Answer key under each item, select the correct answer and set the points.
Click Send, choose the link icon, tick Shorten URL and copy the link. Share it with your class or paste it into your learning platform.
Open the Responses tab to see scores. Click the Sheets icon if you want every answer exported to a spreadsheet.
Tips and common mistakes
Collect email addresses in Settings so each submission is tied to a student.
Preview the quiz yourself with the link before sharing it to catch typos in the answer key.
Shuffle answer options to make copying harder in a room full of students.
Keep one point per item unless a question is worth more, then set it in the answer key.
Questions
Does Google Forms grade the quiz for me?
Yes, once Make this a quiz is on and you set the answer key for each question. Scores appear in the Responses tab.
