What you need
Steps
Open forms.google.com in your browser and sign in with your Google account. Click the blank form option to start a new, empty form.
Click Untitled form at the top and type a clear title, such as your research topic. Add a short description telling respondents what the survey is for and how long it takes.
Click the Add question button to insert your first question. Type the question text, then pick the answer type: short answer, paragraph, multiple choice, checkboxes or dropdown.
Switch on the Required toggle for questions respondents must answer. Keep optional questions off so people are not forced to guess.
Use the Add section button to split a long questionnaire into pages, for example background, main questions and comments. Give each section a short title so respondents know where they are.
Add rating items with the linear scale type, set from 1 to 5, and label both ends, such as Strongly disagree and Strongly agree. Use a multiple choice grid when several statements share the same scale.
Click the eye icon to preview the form and check the layout on both a computer and a phone. Fix any question that looks cramped or unclear before you share it.
Open the Responses tab and click the Google Sheets icon to link a spreadsheet. Every new answer then appears as a row you can sort, filter and analyse.
Click Send to copy the shareable link or email the form directly. To work with a supervisor, open the three dot menu and choose Add editors to invite them by email.
In the Settings tab, turn on Collect email addresses if you need to identify respondents, or limit the form to one response per person. Review the settings, then send the final link.
Tips and common mistakes
Send the form to yourself first and submit a test answer so you can see how it lands in the spreadsheet.
Keep each question short and ask one thing at a time, since double barrelled questions produce unusable data.
Tell respondents how their data will be used and keep the form anonymous unless you genuinely need names.
Check the form on a phone before sharing, because most people will open your link on mobile.
Questions
Do I need to pay for Google Forms?
No. Google Forms is free with any Google account, and the linked responses spreadsheet is part of Google Sheets.
How do I see who answered what?
Open the Responses tab, then the Summary view for charts or the Individual view to read one submission at a time. Linking to Google Sheets gives you the full list of rows.
Can I stop people answering more than once?
Yes. In Settings, turn on Limit to 1 response. Respondents must sign in to their Google account for this to work.
