Steps
Go to forms.google.com and sign in with your Google Account. Click the Blank form box to start a new form, or open an existing one from your list.
Click Untitled form at the top and type a name, such as Photo Contest Entry. Add a line in the description telling people what kind of photo you want.
Click the Add question button, the plus icon on the floating toolbar, to insert a new question.
Open the question type dropdown on the left of the question box and choose File upload. This option only appears when you are signed in to a Google Account.
Type your question in the top field, for example Upload a clear photo of your receipt.
Click the three dot menu at the top right of the question, tick Allow only specific file types, and pick Image so people can send photos only.
In the same menu, set the maximum file size per upload, such as 10 MB, and choose whether to allow one file or several.
Turn on the Required switch at the bottom right of the question if a photo must be uploaded before the form can be submitted.
Click Send, then the link icon, tick Shorten URL if you like, and copy the link to share by email or message.
Check answers in the Responses tab. Uploaded files are saved in a Drive folder that Google creates for the form.
Tips and common mistakes
Respondents must be signed in to a Google Account to upload, so say that in the form description.
Uploaded files count against the form owner's Google Drive storage, so set a sensible size limit.
Fill in the form yourself once before sharing it to check the upload works on a phone.
If you cannot see File upload, your work or school account may have uploads turned off by an administrator.
Questions
Why is the File upload option missing?
You need to be signed in to a Google Account and the form must be one you own or can edit. Some work or school accounts have file uploads disabled by an administrator.
Where do the uploaded photos go?
Google creates a folder in the form owner's Drive named after the form. Each response's files are saved there, and you can open them from the Responses tab.
Can people upload photos from a phone?
Yes. When they open the form link on a phone, the upload button lets them pick a picture from their gallery or take a new one.
