Open Foxit PDF Editor and load your PDF with File > Open, or drag the file onto the window. Check the Form tab is visible in the ribbon.
Click the Form tab and choose Fill & Sign. Foxit highlights the fields it detects so you can see where input is expected.
Click the first field and type your answer. Use Tab to jump to the next field, and tick checkboxes or pick radio buttons as you go.
For a signature, click the Sign option, then choose Place Signature. Draw, type or upload an image of your signature.
Click on the signature line to drop the signature, then drag its corners to resize it so it fits the line.
To build your own form, open a flat PDF and go to Form > Recognize Form Fields. Foxit scans the page and adds fields automatically.
Review the detected fields. Delete any wrong ones and drag the edges to resize boxes so they line up with the printed lines.
Add missing fields from the toolbar: Text Field for names and dates, Check Box for yes or no, Radio Button for one choice, and Dropdown for lists.
Select each field and set its name and properties in the panel, such as required, font size and default value.
Save the file with File > Save As and give it a new name. Open it once more to test that every field accepts input.
Tips and common mistakes
Name your fields clearly, like FirstName or DateSigned, so data is easy to collect later.
Keep the signature image on a transparent background so it sits cleanly on the line.
Run Recognize Form Fields before adding fields by hand, then fix the results instead of starting from scratch.
Test the finished form in another PDF reader to make sure the fields still work outside Foxit.
